Telemedicine has been proven to improve glycaemic management among individuals with type 1 diabetes; however, little is known about the effectiveness of video consultations for those using insulin pumps. In this issue, Schultz et al (https://doi.org/10.1007/s00125-025-06585-2) report the results of an RCT in adults with type 1 diabetes treated with insulin pumps who were randomly assigned to either video consultations or usual in-person care for 1 year. The primary outcome was blood glucose management, measured as time in range. The authors found no significant difference in time in range between the two groups. However, participants in the video consultation group reported better treatment satisfaction and showed a greater reduction in HbA1c, despite a slightly greater negative impact of video consultations on quality of life. The authors conclude that replacing some in-person consultations with video consultations for those with type 1 diabetes using insulin pumps may be feasible while still providing good glycaemic management and potentially improving patient satisfaction.
